We have read, understand and agree with the content of the Decatur Chorus Handbook listed above. We understand that the appropriate Permission to Travel has to be notarized and returned by August 29th for our child to receive full credit for each day of rehearsal in chorus there after. Chorus fee is due in full on or before 9/8/06. We understand that the total financial requirement (the chorus fee) to be in chorus amounts to about 34 cents per day. (Should a parent not be able to meet this financial obligation, it should be communicated to the choral director.) Failure to submit the required signed materials or failure to submit online the required information will result in the student receiving a lowered grade (see course syllabus in the Chorus Handbook) for each six–week grading period until all required signed materials are returned. Grades will not be changed after materials have been submitted for previous six—weeks. We also agree that any funds raised under the auspices of Decatur High School are non--refundable and understand and agree with the trip refund policy stated on the chorus website and in the Choral Newsletter.
